IIO is Seeking Witnesses to a Motor Vehicle Incident in Telkwa (2025-248)
Case: 2025-248 Incident Date: October 26, 2025 Agency: Special Provincial Constable Location: Telkwa Incident Classification: Serious Harm Case Status: Open - Investigation in ProgressSurrey, B.C. — The Independent Investigations Office of BC (IIO) is investigating a motor vehicle incident that occurred on October 26, 2025 in Telkwa.
Information provided by the BC Conservation Officer Service states that shortly before 10 a.m., COS members observed a pickup truck matching the description of a vehicle being driven erratically that had been reported to the RCMP.
The burgundy-coloured Dodge pickup truck was travelling westbound on Highway 16.
The COS officers conducted a traffic stop on Highway 16 near Walcott Rd., but the man driving the pickup drove away shortly after stopping.
A brief time later, COS officers located the pickup truck at the bottom of an embankment off Highway 16 in Telkwa, with the man who had been driving still inside.
Officers provided aid and called the fire department, RCMP and BC Emergency Health Services for assistance.
The man was taken to the hospital for assessment and treatment.
The IIO was notified of the incident shortly after it occurred and is now investigating.
Investigators are asking anyone who witnessed the initial traffic stop or saw the pickup immediately prior to it leaving the road, to contact the IIO.
There are two factors that the IIO considers at the beginning of each investigation:
- If there has been an injury that meets the threshold of serious harm, as defined by the Police Act, or a death; and
- If there is a connection between the serious harm/death and police action or inaction.
If both conditions are met, the investigation will continue to examine all available evidence to determine what occurred. At the conclusion of the investigation, the chief civilian director will consider whether there are reasonable grounds to believe that an offence may have occurred.

Background
The IIO is the independent civilian oversight agency of officers and detention guards in British Columbia. It investigates all officer-related incidents that result in serious harm or death, whether or not there is any allegation of wrongdoing.
