Surrey, B.C. — The Independent Investigations Office of BC (IIO) is seeking witnesses to a fatal officer-involved shooting that occurred in North Vancouver on the afternoon of July 21, 2026.

The IIO is asking anyone who witnessed the incident or who is in possession of video footage – including from cell phones or vehicle dashboard cameras – to contact the IIO.

Initial information provided by the RCMP states that shortly after 1 p.m., officers responded to reports of a man in possession of a weapon who was inside a business located in a commercial plaza in the 2600-block of Westview Drive.

It is reported that the man left the business and entered a vehicle in the parking lot before being involved in a collision with other vehicles parked nearby.

Responding officers encountered the man at a gas station in the same plaza. At one point, it is reported the man attempted to enter a police vehicle, leading to an interaction with officers. During the interaction, shots were fired by police and the man suffered injuries.

The man was taken to the hospital by ambulance for assessment and treatment but was later pronounced deceased.

The IIO was notified of the incident shortly after it occurred and has commenced an investigation.

There are two factors that the IIO considers at the beginning of each investigation:

  1. If there has been an injury that meets the threshold of serious harm, as defined by the Police Act, or a death; and
  2. If there is a connection between the serious harm/death and officer action or inaction.

If both conditions are met, the investigation will continue to examine all available evidence to determine what occurred. At the conclusion of the investigation, the chief civilian director will consider whether there are reasonable grounds to believe that an offence may have occurred.

Background

The IIO is the independent civilian oversight agency of officers and detention guards in British Columbia. It investigates all incidents involving officers or detention guards that result in serious harm or death, whether or not there is any allegation of wrongdoing.
